Please complete the enrolment form below. This form will require 5-10 minutes to complete.

To help complete the enrolment form quickly, we recommend having key information available such as; your current address, the address you’ll be living at whilst studying, your emergency contact or next of kin, past study information.

Alternatively you can request a pdf document version of the enrolment form which can be printed and completed in written form. Please email info@excel.ac.nz to request a printable version.

*A verified copy is a copy of the original that has been signed by an authorised person. This person could be a Justice of the Peace, Barrister, a court register or local authority employee designated for this purpose. You can locate a Justice of the Peace here.

International students must have a current student visa or permit to study at EXCEL School of Performing Arts.

ADMISSION

An admission fee of $150 nzd is required to secure your placement at EXCEL School of Performing Arts. This fee is non-refundable. Payment can be made via bank transfer, cash or via eftpos.

DECLARATION

PRIVACY ACT 1993 – EXCEL collects and stores information from this form to comply with the requirements of the Ministry of Education (funding and student statistical returns), New Zealand Qualifications Authority (Record of Learning registration and Unit Standards outcomes), Education and Training Support Agency (funding and student statistical returns), Industry Training Organisations (funding and academic outcomes) and Studylink (confirmation of enrolments and academic outcomes). The information is also used to select students for programme, to manage internal administrative processes, and for internal reporting. Information about students may be supplied to, and sought from, other educational institutes for the purpose of verifying academic records. In addition, when required by statute, the Institute releases information to Government agencies such as the New Zealand Police, Department of Justice, and Studylink

In signing this enrolment form you authorise such disclosure on the understanding that the institution will observe the general conditions governing the release of information, as set out in the Privacy Act 1993. You may see any information held about you and amend any errors in that information. To do so, contact one of our office staff.

FEES – In signing this enrolment form you undertake to pay all fees as they become due. The Institute’s policy on withdrawal and refund of fees is as per Government legislation policy which can be obtained from the Excel prospectus.

RULES – In signing this enrolment form you undertake to comply with the published rules and policies of Excel School of Performing Arts with regard to attendance, academic progress, standard of dress, health and safety, and behaviour.
